Fee for Birth or Death Certificates

• $18 for a birth certificate
• $16 for a death certificate from Davis County
  (Note: To avoid the additional $12 fee for a death certificate for another county, please consider getting the certificate directly from the county where the death occurred.)
• $8 for an additional certified copy of either birth or death certificate (that are requested for the same record on the same day)
• $7 for a burial transit permit
• $15 for a stillborn birth
