Food Handler Permits

Individuals working in facilities that prepare or serve food are required to obtain a food handler permit from the Davis County Health Department’s Environmental Health Services Division. Permits are obtained by passing an examination based upon the Davis County Health Department "Food Safety Training Manual." You may download the English-language study manual (pdf) or purchase one from the Food & Facilities Bureau office for $2.00.

If you need to take the test in Spanish, you can download the Spanish-language study manual (pdf) or purchase a copy from our bureau for $2.00.

Tests are given between the hours of 9 a.m. and 4 p.m. during normal working days. Currently the fee for a permit is $20. The permits issued are photo ID cards that must either be worn or carried on the person while working in a food establishment.
