The Davis County Health Department offers all required and recommended immunizations for school.

Seventh Grade Entry Requirements 2022 -2023 PDF - English / PDF - Spanish

2 Measles, Mumps, Rubella (MMR)
3 Hepatitis B
2 Varicella (Chickenpox) - history of disease is acceptable, parent must sign verification statement on school immunization record
1 Meningococcal
4 Polio
2 Hepatitis A
5 DTap/DT

Kindergarten Entry Requirements 2022-2023 PDF -English / PDF - Spanish
5 DTaP/DT - 4 doses if 4th dose was given on/after the 4th birthday
4 Polio - 3 doses if 3rd dose was given on/after the 4th birthday

Early Childhood Programs Requirements 2022-2023 PDF - English / PDF - Spanish
Children attending early childhood programs (those in a licensed day care center, nursery or preschool, child care facility, family home care, or Head Start program) are required to be immunized appropriately for their age with the following immunizations:
Hepatitis A
Hepatitis B
Pneumococcal
Varicella (Chickenpox)
Polio - 3 doses if 3rd dose was given on/after the 4th birthday
Haemophilus infuenzae type b (Hib)
DTaP
Measles, Mumps, Rubella (MMR)
Recommended Yearly Vaccination
Flu shots are recommended to everyone 6 months of age and older every season.
